What is Social Support Group

Social Support Groups aim is to improve a person’s wellbeing by reducing the risk of isolation.
They support independence and encourage keeping active.
The benefits are improved quality of life and general wellbeing preventing early admission into Aged Care.

What we offer

We provide a social half day in a small friendly with a range of activities which you can enjoy. A hot meal is offered as well as afternoon tea.

Social Support groups provide you with opportunity to remain connected with your community and relieve boredom and loneliness.

We provide regular outing exploring the local area in the Community Bus.

Attendance

The Social Support Group runs on a Thursday. If you regularly participate and are unable to attend a group, we ask that you notify staff before 10.30 on the day.

Fees

There is a small fee required to attend the Social Support Group to enable its continuation.

Fees are:

Program Fee          $10.00

Meal Cost               $10.00

Bus Fee                   $2.00

Outings incur any additional costs associated such as entry fees, cappuccino.
Meal fee does not apply for lunch outings instead it will be the cost of what you order for lunch.
We aim to keep costs to a minimum.

Social Support Groups are largely funded by:
Commonwealth Home Support Program (CHSP)
Home and Community Care (HACC – PYP)

Eligibility / Access

Access to services is through My Aged Care.

“My Aged Care is the starting point to access Australian Government funded aged care services.
The phone line and website can help older Australians, their families and carers to get the help and support they need”.

My Aged Care can assess eligibility for subsidised services to enable people to remain living at home longer.
If your 65 years or older (or 50 years for Aboriginal and Torres Strait Islander people) you will need to contact
My Aged Care on; 1800-200-422 or through their website; www.myagedcare.gov.au to register.
The Centre is administered by the Cohuna District Hospital under the guidance of a qualified
Social Support Coordinator and Assistant, who are helped by a team of in-house volunteers and
volunteer drivers from the community.

Transport

Our bus is fitted with a hoist for clients in wheelchairs and wheelie frames and there is a staff member is on board at all times to assist clients.  We can take you to and from your home to the Centre.
You are welcome to make your own way if you prefer to do so.
