Advance care planning involves thinking about, discussing with your family and close friends, and possibly documenting what types of health care you may or may not wish to receive – if you become seriously ill and/or unable to speak for yourself.

The District Nursing Service is a home based nursing service providing care in the community. Cohuna District Hospital offers a daily service, between 8am and 4.30pm Monday to Friday.

All our clients are assessed frequently by staff with regular communication between local general practitioners and their nephrologists.

Cohuna District Hospital (CDH) maternity services has been assessed against the Victorian Maternity and Newborn capability framework and is a Level 2 Service.

The Home was opened in 1985 as a result of a successful fundraising program in the townships of Cohuna, Leitchville, Gunbower and surrounding districts.
